i doubt that one cigarette (i'm assuming) it going to do anything to you, although technically i suppose it could. Its certainly not a healthy habit, CF or not, to pick up so i wouldn't recommend continuing. And since you are in such good health it would be a shame to do something to jeopardize...

I did not mention it on any of my applications. but once accepted to the school i wanted to go to i did contact the health center and just let them know my situation so that if i were to show up sick they would have some clue as to what was going on. unfortunately, though they were super nice...

maybe upping the number of treatments you do during the day? i think the best option though would be to talk to your doctor and see if there is an oral antibiotic, tobi, colistin etc. that you could try before IVs.
