I don't know anything about you, but my best fev1 lately was 41 and I was out in 19 degree weather 2 days ago and didn't have a problem like you stated.
I would get ahold of your doctor.

This may be impossible, but cover your face with a scarf or balaclava, and breathe through your nose. Those ten degree days will be minus ten soon. This is how we were taught to survive more extreme weather than ten below. Fulling breathing, in and out through the nose warms and humidifies air going into the lungs and conserves energy exhaling. It is like brain freeze with ice cream only it is the bronchial tubes. Your soft lung tissue has nerves that refer pain to the ribs or back so that may help narrow down what is hurting from the cold.

I absolutely get sharp pains in my lungs during winter time. Part of the problem is the coldness, but mostly it is the dryness. I think the dryness makes parts of my lungs stick together, which hurts when I try to inhale.

My solution: I do have a humidifier (a Venta Airwasher to reduce bacterial contamination). But now I also wear disposable surgical masks when I sleep. The mask traps some of the humidity every time I exhale, which seems to really help with dryness.

When I go outside, I always cover my face and breathe through a scarf, this traps humidity and also keeps the air I am breathing warmer.

Prior to using the masks, I had to keep the humidity in my place very high to avoid lung pains. Obviously this is bad news as bacteria will grow well. The masks seem to help more than just humidifying alone.
