Altera and Tobi anyone?

So I was at clinic on Monday and asked about getting compounded Tobi, which they didn't have a problem with. I stated I already had a box of Tobi Novartis brand Tobi.
The NP said I could take HALF a vial of Tobi brand Tobi and use it in the altera and it would be the same as compounded tobi.
However I know Foundation Care and the instructions say not to use ANYTHING but Cayston in the Altera.
I didn't think about it until later, but is the NP confusing the Altera with the E-Flow? I know the aerosole heads are different between the two, would this cause any problems using the Cayston Aerosole head with Tobi? I can't really see the issue but figured someone may know.

my doc told me that the mesh in the altera is a little more fine than in the eflow/trio so that the med particles end up being smaller.
that being said, i do everything in the altera and am doing just fine. the heads fit interchangeably in the clear plastic chamber part so if you have an eflow/trio head you can just put it in the altera clear chamber part and be good to go.

I don't have an eflow head, just the altera, which is why I ask. I guess smaller the Tobi particles the better? That's pretty much what I have figured.

I was told not to use TOBI in the TRIO only reformulated Tobra. Which I know you know. I personally would not fool around with putting half of TOBI . I mean how do you know you really got half. I have used the TRIO and Alterra and me too have used the Alterra for reformulated Tobra and use the TRIO head just for Colistin.

I do not use any of it anymore cause the heads just clog no matter how GREAT I am with cleaning and it is a pain. I also feel TOBI is more effective for me then reformulated and I like the TOBI in the pari lc plus nebs. I just feel it is all more effective for me. Just my opinion and my situation. So I stopped all the TRIO and Alterra. Cayston made me feel worse but was great in the beginning. Now I am on TOBI every day, every month and seems to be working well or at least better than Cayston and Coli. Doc says inhaled TOBI does not cause resistant issues like through IV's. So i am going with that!

I use both compounded Tobra (with Trio metal piece) and Cayston (with Altera metal piece). I was told that I could use HTS or Pulmozyme in the Trio head, but it would be more like half of the vial. They said it wouldn't hurt to use the whole vial, but half a vial is sufficient.

I am not sure about using the Altera head, but I was also told to ONLY use Cayston with that head (per IV Solutions). But who knows - sometimes your clinic knows more than the pharmacies. For example, my clinic tells patients that they can do Cayston in 1 or 2 treatments per day versus the normal 3 (by combining vials). I know that the pharmacy would never tell you that, but the clinics will.

If I were you, I would just verify with the NP that she meant the Altera piece versus the Trio piece. If she says it's still ok, then go for it!
